Missing Physician Order for Midline IV Catheter

During observation, the resident’s left upper arm midline catheter was in place, and the site base had brownish discoloration with a small darkened blood-tinged area in the tubing. Record review showed the resident had orders for oral antibiotics only, including levofloxacin and doxycycline, and there were no IV antibiotics ordered during the facility stay. The physician order sheets, MAR, and TAR for November and December 2025 did not contain orders for discontinuance, care, or maintenance of the midline catheter. The resident stated she had had the catheter since admission and did not know why it was still in place, and said she had not been receiving medication through it since admission. The catheter remained in place and unused for 11 days, with repeated observations on 12/08/25 and 12/09/25 showing no current physician order for removal or for care and maintenance. Staff B acknowledged that no physician order had ever been obtained for the catheter’s care and maintenance, and Staff A stated she had noticed the IV in place but did not notify the oncoming nurse, DON, or the physician. The DON acknowledged that the nurse should have contacted the physician to obtain an order to discontinue or provide for care and maintenance of the midline IV catheter.

However, the Nursing Admission/readmission Evaluation/assessment did not document the measurement and assessment of the PICC line, and the record did not show evidence that the length of the catheter above the insertion site or arm circumference were obtained upon admission. The facility also failed to ensure Resident 123’s IV tubing was labeled. During an initial tour, Resident 123 was observed with a peripheral IV in the left hand and a one-liter bag of 0.9% Sodium Chloride infusing at 75 mL per hour. The IV tubing was not labeled with the date, time, and nurse’s initials. The resident’s order summary showed an order to change IV tubing every 24 hours for intermittent IV infusion and TPN and every 72 hours for continuous IV infusion. In addition, the facility failed to ensure Resident 9’s IV medication was correctly labeled. During medication administration observation, Resident 9 was observed with a right upper arm three lumen PICC line and meropenem 1 gram was prepared and administered through the PICC line. The IV tubing was labeled by the RN, but the meropenem medication bag was not labeled with the date, time, and initials of the RN who administered the medication. Facility policy stated that the nurse hanging IV solutions will label the bag with the date, time, and initials.

During interview and record review, RN 1 confirmed the resident was receiving IV antibiotics through the midline and stated nursing staff were responsible for dressing changes and measurements, but could not verify any baseline measurements from the acute care hospital. The DON later reviewed and verified these findings.

The DON stated PICC lines are to be changed every 7 days and acknowledged the dressing should have been changed the day before.
