Summary

The facility failed to store, prepare, distribute, and serve food in accordance with professional standards for food service safety for 1 of 1 kitchen areas reviewed. During observation on 02/10/2026, a crate of individual milk boxes was found on the floor under shelving in the kitchen refrigerator. During interview, the DM stated items in the refrigerator should be 8 inches from the floor and 16 inches from the ceiling, and stated the risk of leaving items on the floor could include mold, a dirty floor, or critters getting into the items. The DM also stated items are labeled with a date so staff know when they were opened and when they are getting ready to expire. On 02/12/2026, observation of the nourishment refrigerator/freezer revealed one cottage cheese container with no seal and no lid, with no open date or resident name; one sealed cottage cheese with no resident name; three homestyle chicken noodle soups with no resident names; one chicken soup with no resident name; and one frozen cheese enchiladas meal with no resident name. The DON stated items in the nourishment refrigerator/freezer should have the resident's names on them and, if opened, the date when the item was opened. The DON stated the risk could be someone else taking the item and that items without dates would be thrown away. The DDM stated there was no specific policy regarding the nourishment refrigerator/freezer, and the Administrator stated the dietary department was responsible for the nourishment refrigerator. The facility policy, Food Storage: Cold Foods, revised 2/2023, stated all food items will be stored 6 inches above the floor and 18 inches below the sprinkler unit.
