Interviews reflected that staff knew the freezer was not working, but there was confusion about who was responsible for checking temperatures, reporting the problem, and discarding food. The maintenance supervisor stated the vendor had identified a frozen evaporator coil and later a controller issue, and the dietary manager stated the freezer had been known to be not working since Friday.
