Summary

Drugs and biologicals used in the facility were not labeled in accordance with currently accepted professional principles for 1 of 3 medication carts reviewed, the A-hall medication cart. During an observation and interview on 4/10/2026 at 7:46 AM, a vial of insulin was found in the top drawer of the A-hall medication cart without an expiration date. The LVN was unsure when the vial had been opened and stated that insulin vials and pens should be labeled with the date they are opened and the date they expire so residents do not receive expired medications. In an interview later that morning, the DON stated the facility policy is to label all insulins with the date they are opened and the date they expire, and that nursing staff were primarily responsible for this task. Record review of the facility's Storage of Medication policy dated 10/3/2018 showed that refrigerated medications must be stored in a secured location and labeled accordingly, but the policy did not include guidance for labeling medication to indicate expiration.
