During interview on 1/7/2026, the scheduler stated that if a call-in occurred during working hours, the scheduler would find a replacement, but if a call-in occurred after hours, on weekends, or on holidays, the on-call person would be responsible for finding coverage. The scheduler later stated that an RN had been scheduled for the night shift on 12/14/2025, but the RN called in and the on-call staff replaced the shift with an LPN without confirming whether another RN replaced the shift or whether an RN had worked previous shifts. The surveyor shared the concern with the NHA and DON, and no further information was provided at the time of the write-up.

The facility license showed a 64-bed skilled nursing facility, and the resident roster dated 7/22/26 showed 55 residents residing in the facility, all requiring skilled nursing services.
