Summary

Surveyors identified a deficiency related to the resident right to a safe, clean, comfortable, and homelike environment in one of two resident shower rooms on the [NAME] Fort Hall. During observation of the central shower room on the 300-400 Hall, the surveyor noted a damaged area of the wall that had been covered with black tape, as well as stained and discolored tile grout in the shower stall. These environmental issues were directly observed by the surveyor on the same day and were subsequently verified in an interview with the facility’s Director of Plant Maintenance. The facility Administrator also acknowledged these findings during the on-site observation and again at the exit conference.
