Failure to Provide Required QAPI Training
Summary
Mandatory training on the facility’s Quality Assurance and Performance Improvement (QAPI) Program was not provided to all staff. Based on interview and record review, the facility failed to develop, implement, and maintain an effective training program for 5 of 16 staff reviewed for QAPI training, including the DON, DM, RN C, LVN E, and NA G. Personnel records showed each of these employees had a hire date in 2025, but there was no evidence that they completed QAPI training upon hire or while working at the facility. During interviews, the ADMN stated new hires were given orientation through a hyperlink and that annual training was done through monthly in-services, but he needed to look for the training required for the employees in question. The AIT stated department heads were responsible for ensuring training was completed, with the office manager responsible for administration staff, and that the ADMN and office manager monitored completion. She stated the QAPI training was not done due to an oversight by the ADMN. The DM stated she did not remember receiving any orientation training on QAPI and could not remember ever receiving QAPI training. Facility policy required newly hired personnel to attend a 10-hour orientation within their first five days of employment and maintain a written record of orientation, but the personnel records reviewed did not show QAPI training for the staff in question.
