Record review showed that the DON, DM, RN C, LVN E, NA G, and NA H all had hire dates documented in their personnel records, but there was no evidence that any of them completed effective communication training upon hire or while working at the facility. During interviews, the ADMN stated that new hires were given orientation through a hyperlink and that annual training was provided through monthly in-services. He also stated he would look for the training required for the employees in question and later stated he was responsible for ensuring training was completed for all employees. The AIT stated that each department head was responsible for ensuring employee training was completed, and that the ADMN and office manager monitored completion. She stated the training was not done due to an oversight by the ADMN. The office manager was not available for interview. The DM stated she did not remember receiving orientation training on effective communication and could not remember ever receiving that training. Facility policy for newly hired employees required a 10-hour orientation program within the first five days of employment and a written record of each employee's orientation program. The facility's personnel policies and staff development policy did not include information on required effective communication training during orientation or annually.

During an interview on 4/19/26 at 1:50 p.m., Human Resource Employee E9 confirmed that the facility failed to provide annual training on Effective Communication for these two staff members.

The HR Director also stated there was currently no Nurse Educator or Staff Development Coordinator. The NHA and Director of Operations confirmed the facility had no evidence of communication training being provided and stated training records were in boxes kept by the previous DON, with no organized system for employee trainings.

Review of the sheets showed no documentation of the subject matter covered. The Administrator also stated there was no agenda followed for the monthly in-services, that topics were provided according to a schedule, and that the in-services were done from memory. He further stated the facility did not have documentation of what educational material was presented at the monthly staff meeting and could not ensure each staff member received the training.
