The DON and administrator confirmed the hand-off report indicated oxygen at night, acknowledged there was no current physician order for oxygen, and stated staff should have clarified unclear details with the physician. The facility also had no policy regarding nurse hand-off reporting, and its oxygen administration policy required verification of a physician order specifying route, liter flow, and duration.
