Summary

The facility failed to ensure proper use of PPE for residents on contact precautions and Enhanced Barrier Precautions (EBP). For one resident on contact precautions, the DON was observed wearing gloves while providing care, then exited the room with the gloves still on, walked to a medication cart outside the room, removed the gloves there, obtained a new pair, and returned to the room. The DON later acknowledged not following proper PPE use and hand hygiene for that resident. For another resident on contact precautions, a CNA was observed at the bedside with no PPE in use despite signage at the room entrance stating contact precautions were required. For a third resident on EBP, the room sign directed staff to wear gloves and a gown for high-contact care activities, and a staff member was observed at the bedside holding linens while wearing gloves during resident care. The DON and Infection Preventionist stated that for EBP staff were expected to use gowns and gloves during direct care, and for contact precautions staff were expected to apply gloves, a gown, and a mask before entering the room and remove PPE before exiting. The facility also failed to report an outbreak of infectious dermatitis to the state health authority. The infection control surveillance record identified 18 residents with skin infections/rash and 8 additional exposed residents, for a total of 26 affected residents across multiple hallways. The IP and DON stated the condition began with a red rash and itchiness on multiple body areas, that the Medical Director diagnosed it as infectious dermatitis, and that residents were placed on contact precautions and treated. The IP acknowledged that more than one case constituted a reportable outbreak, but the facility only emailed the state infection prevention contact for guidance, received no response, made no follow-up contact, and did not report the outbreak.
