During an interview on 1/7/26 at 11:50 AM, the Director of Nurses (DON) confirmed that the facility did not have eight hours of RN coverage every day and acknowledged the need to hire more RNs to meet the requirement. The facility assessment dated [DATE] documented that an RN is needed every day to provide competent support and care for the resident population, and the Central Management Services 802 Matrix dated 12/23/25 confirmed the facility census of 107 residents. The deficiency centers on the facility’s failure to ensure daily RN coverage consistent with its own facility assessment and regulatory requirements, as evidenced by documented staffing schedules and the DON’s confirmation of inadequate RN staffing on the identified dates.
