Food Service Equipment and Dinnerware Not Kept Clean

Summary

The facility failed to follow proper food safety sanitation standards and food handling practices during kitchen and nourishment room observations. On 11/19/2025 at 11:00 AM, surveyors observed cracked plates, cracked bowls, and meal trays with rusted, cracked edges on the tray line and dish rack across from the reach-in refrigerator. The meat slicer on the bottom shelf of the prep table was observed with food debris on and around the blade, and one of two microwaves in the 300-600 unit nourishment room was observed with food debris stuck on and around the inside of the microwave. During interviews on 11/20/2025, staff gave differing accounts of who was responsible for cleaning the meat slicer and the microwaves in the nourishment rooms. One Dietary Aide reported that all kitchen staff were responsible for cleaning kitchen equipment after each use and that Dietary Aides were responsible for washing dishes and watching for broken and rusted dinnerware and utensils, while the Dietary Manager was responsible for cleaning the microwave ovens in the nourishment rooms. Another Dietary Aide reported that cooks were responsible for cleaning the meat slicer after each use and stated Dietary was not responsible for cleaning the nourishment room microwaves. The Food Service Director reported that Dietary Aides were responsible for running the dish machine and reporting concerns with dinnerware and utensils, cooks were responsible for cleaning equipment such as the meat slicer after each use, and was unsure who was responsible for cleaning the microwaves in the nourishment rooms.
