Insulin Pen Used for the Wrong Resident

Berkley West Healthcare CenterSanta Monica, California Survey Completed on 01-08-2026

Summary

The facility failed to ensure that insulin intended for single-patient use was ordered and delivered in a timely manner for one resident. During a concurrent observation and interview of medication administration, an LVN injected Resident 34 with 2 units of insulin via insulin pen for a blood sugar result of 204, but the label on the insulin pen showed that it was prescribed for Resident 46. The LVN stated she accidentally used Resident 46's insulin pen to administer insulin to Resident 34. She also stated that Resident 34's insulin pen was in the medication cart, but none was found in the medication cart or the medication refrigerator for Resident 34. During interview and record review, the DSD stated that a renewal order for Resident 34's insulin pen had been completed the prior night, but the facility's contracted pharmacy had not delivered the medication to the facility. The DON stated that sharing a single-patient insulin pen with any resident other than the resident for whom it was prescribed can lead to reduced intended time use of the prescribed dose for the intended resident. Facility policy stated that medications supplied for one resident are never administered to another resident, and the LVN job description stated to ensure that prescribed medication for one resident is not administered to another.
