Summary

The facility failed to ensure an accurate Level I Preadmission Screening and Resident Review (PASRR) was completed for one resident. Upon review of the resident's admission record, it was found that the resident had a diagnosis of schizophrenia and was prescribed aripiprazole, an antipsychotic medication. The resident's Minimum Data Set (MDS) also indicated severe cognitive impairment and a need for assistance with most activities of daily living. However, the Level I PASRR screening completed prior to admission did not identify the resident's serious mental illness or the use of psychotropic medication.
