Failure to Provide Scheduled Shower Assistance
Summary
The facility failed to ensure that two residents received staff assistance with activities of daily living, specifically weekly showers, as scheduled. R3 had diagnoses including CHF, diabetes, fibromyalgia, chronic pain, weakness, and difficulty walking, and was receiving Hospice services. R3’s MDS showed intact cognition, impairment in both upper and lower body function, partial/moderate assistance needed for toileting, and full dependence for upper/lower body dressing and footwear. R3’s care plan and Kardex indicated bathing/showering assistance of one staff and a Sunday evening shower schedule, yet R3 reported not receiving regular showers, stated the facility was understaffed, and said R3 had gone three weeks without a shower and had a fungal rash under both breasts. Record review for R3 showed inconsistent shower documentation and multiple missed scheduled showers from August through December 2025. The task logs reflected that only 7 of 20 scheduled showers were completed during that period, with several entries showing no documentation or refusals without corresponding progress notes. Hospice staff confirmed Hospice was not providing bathing assistance because R3 had requested facility staff to do so, and the Hospice order reflected that request. Surveyor interviews with R3 on multiple occasions confirmed that R3 still had not received a shower during the survey period. R41 also had intact cognition on MDS and diagnoses including dementia, CHF, chronic kidney disease, type 2 diabetes, weakness, anxiety, and depression. R41’s MDS indicated the resident required supervision/touch assistance with bathing and varying levels of assistance with dressing and footwear. The care plan and Kardex indicated one-staff assistance for bathing/showering and a Monday shower schedule, but R41 reported not receiving enough showers and stated it had been at least two weeks since the last shower. During observation, R41 told CNA-I that a shower had not been provided as scheduled and did not want to wait another week. Task logs showed only 6 of 20 scheduled showers completed for R41 between August and December 2025, with many scheduled showers lacking documentation or being marked refused without progress note documentation. Facility leadership stated residents should receive showers as scheduled, refusals should be documented, and showers should be re-offered, but the records reviewed showed repeated missed showers and incomplete documentation for both residents.
