Summary

The facility did not use the services of a registered nurse for at least 8 consecutive hours a day, 7 days a week. Review of staffing schedules from September 2025 to October 2025 showed that no RN was scheduled on 9/14/25 and no RN was scheduled on 9/28/25. During interviews on 10/8/25, the DON stated that the facility did not need to have an RN scheduled on every shift and said that if residents needed an assessment completed, the nurse would call her. The Administrator stated that if residents needed intravenous therapy or a wound vacuum, they would be sent out, and said they were not sure whether it was necessary to have an RN on every shift based on the type of facility. In a follow-up interview, the Administrator confirmed that only an LPN worked in the facility on the 14th and 28th of September and that no RN worked in the facility on those days.
