Summary

The facility failed to ensure safe and appropriate respiratory care for two residents who needed oxygen therapy. One resident with diagnoses including COPD, pneumonia, and hypoxia was observed lying in bed with a nasal cannula in place and oxygen running at 2.5 LPM, but there was no physician order for oxygen in either the electronic chart or paper chart. During interview, the LVN confirmed there was no oxygen order and stated the resident needed oxygen to keep saturations above 90%; the DON also stated that a physician order was important because nurses must follow physician orders and without one staff would not know how much oxygen to give. A second resident, a 76-year-old female with CHF, acute respiratory failure with hypoxia, dementia, diabetes, rhabdomyolysis, and hypertension, had a physician order for continuous oxygen at 2-4 L via nasal cannula every shift for shortness of breath. However, when observed in her room, she was not receiving oxygen and there was no concentrator present. Her O2 saturation was checked and read 84%. The LVN stated she was not sure if the resident was on oxygen and was not aware of the physician order, and the DON stated nurses were responsible for following physician orders and did not know how long the resident had been without oxygen.
