Summary

The facility failed to ensure Resident #6 had the call light within reach while in bed. Resident #6 was a [AGE]-year-old female admitted on 12/06/22 with diagnoses including encephalopathy, dysphasia following cerebral infarction, muscle wasting and atrophy, history of falling, and lack of coordination. Her quarterly MDS dated 06/20/25 showed a BIMS score of 2, indicating severe cognitive impairment, and she used a wheelchair. During an observation on 07/21/25 at 9:45 AM, Resident #6's call light was pinned to the bed sheet at the top edge of the bed, and she was unable to reach it. During an interview, LVN B observed the call light pinned at the top edge of the bed and stated the resident was supposed to have the call light close to call for help. LVN B also stated that all residents were to have their call lights accessible and that not having it near could have resulted in an unnecessary fall or injury. The DON stated that nurses and CNAs must check residents frequently and ensure call lights are within reach, and that not having the call light within reach could have resulted in an unnecessary fall. The facility policy titled Answering the Call Light stated that when a resident is in bed or confined to a chair, the call light should be within easy reach.
