Summary

The facility failed to post daily nurse staffing and census information as required for 13 out of 15 days reviewed. Observations on 12/29/2025 revealed that the most recent posting was dated 12/16/2025, and interviews with staff indicated confusion and lack of clarity regarding responsibility for updating and posting this information.
