Infection Control Failures With Oral Stimulator Handling and Room Cleaning

Summary

The facility failed to maintain an infection control program designed to provide a safe, sanitary, and comfortable environment on one unit. Resident #122 had an oral stimulator used for chewing and swallowing support, but it was observed hooked to the resident’s wheelchair by a lanyard and touching the wheelchair seat while the resident was sleeping. At another observation, two unidentified staff members offered the oral stimulator to the resident using bare hands after clearing dishes from the dining room, and hand hygiene was not performed before touching the device. The resident was seen placing the oral stimulator in her mouth. Staff interviews showed the speech therapist had written for the device in July and August 2025 and discontinued it in August 2025, while the DOR stated he was unaware the resident was still using it and would not recommend attaching it to the wheelchair. The infection preventionist stated there were infection control issues with multiple staff touching the oral stimulator after removing dirty dishes and not washing their hands. The report also documented failures during room cleaning by housekeeping staff. During a continuous observation in a resident room, Housekeeper #1 and Housekeeper #2 cleaned surfaces including windows, the bed, dresser, sink, vanity, toilet, and floor. Housekeeper #1 repeatedly removed soiled gloves and donned clean gloves without performing hand hygiene. Housekeeper #2 also changed gloves without hand hygiene. The staff sprayed surfaces with Oxivir Five 16 concentrate solution and immediately wiped them down rather than allowing the disinfectant to remain on the surface. The facility’s own policy required hand washing before and after glove use, and the product insert for Oxivir Five 16 stated the solution must remain on surfaces for a minimum of five minutes. The maintenance supervisor confirmed the dwell time was five minutes, and the DON stated hand hygiene should be completed before and after donning gloves and that the dwell time was important for effective cleaning. The observations showed the cleaning process did not follow those requirements.
