Summary

The facility failed to complete a quarterly MDS assessment for one resident within the required 3-month timeframe. Resident #92, a male with diagnoses including Alzheimer's Disease, Major Depressive Disorder, psychotic symptoms, cognitive communication deficit, and muscle weakness, had a quarterly MDS that was not completed on time. Record review showed the resident’s last completed MDS was on 04/19/2025, and the assessment was not processed until 08/22/2025, totaling 125 days between assessments. The resident’s care plan dated 06/18/2025 showed he was dependent on staff and required assistance and verbal encouragement with mobility, bathing, hygiene, toileting, dressing, grooming, eating, and other ADL care needs while being encouraged to maintain independence. During interviews, the DON stated the facility did not have an MDS Coordinator and that a Regional MDS Coordinator was covering MDS duties. The RMDS Coordinator stated he had not received facility in-service training, had not checked back dates for MDS completion, and missed the resident’s MDS. He stated there was no MDS Coordinator onsite as of 08/08/2025 and that he was responsible for monitoring MDS timeframes. The DON, RMDS Coordinator, and ADM all stated the resident’s MDS was missed or late, and the facility records showed no MDS in-service trainings or education from January 2025 through August 2025.
