Scheduled Activity Not Held

The Meadows On Sunset Post AcuteLos Angeles, California Survey Completed on 11-18-2025

Summary

The facility failed to implement its activities program for one of seven sampled residents when the resident activity room was observed closed at 2 p.m. and no activities were being held, even though the facility activities calendar indicated that Crossword Club was scheduled at that time. Resident 2 was admitted on 6/1/2022 and readmitted on 1/19/2024 with diagnoses including essential hypertension, generalized muscle weakness, and major depressive disorder. The resident’s MDS dated 10/2/2025 indicated the resident had the ability to understand and be understood. During interview, Resident 2 stated the resident had been told at the resident council meeting that the facility would be doing a weekly calendar for the remainder of the month and that December would have a full calendar of activities. Resident 2 also stated the October calendar was still posted in the room, that there were no activities that day, and that the resident would like some sort of activity because the resident enjoys participating and interacting with other residents. At the time of observation, the activity room was empty with no residents or staff present. Staff interviews confirmed there were no activities at 2 p.m., and the DSD stated that if the facility scheduled activities but did not do them, the facility failed to fulfill residents’ expectations and failed to provide the services to meet residents’ needs.
