Failure to Perform Hand Hygiene During Perineal Care

Summary

The facility failed to ensure proper hand hygiene during perineal care for two residents observed during bowel and bladder care. For one resident, two CNAs assisted with brief care, cleaned the front perineal area and buttocks, and then one CNA opened the bedside drawer and handled ointment and returned wipes to the drawer without changing soiled gloves. The resident had diagnoses including stroke, type 2 diabetes mellitus, and protein malnutrition, and required total care for toileting, personal care, and lower-body dressing, with maximum assistance for bathing. For the second resident, two CNAs provided perineal care while using a sit-to-stand lift. One CNA removed the wet brief, wiped the buttocks, front skin folds, and urinary meatus, then stored wipes in a drawer and pulled the resident’s clothing up before both CNAs removed their dirty gloves. The resident had diagnoses including dementia, overactive bladder, and ataxia, was severely impaired on the MDS, required total care, and was always incontinent of bowel and bladder. During interviews, CNAs stated they were trained to wash hands and wear gloves before perineal care and to remove gloves and wash hands when done, and both acknowledged gloves should have been changed during care to prevent cross contamination. They also stated there was no time to change gloves during care with closed unit residents and that they had touched the lift, clothing, wheelchair, and drawer while wearing dirty gloves. The DON confirmed hand hygiene was needed between clean and dirty tasks to prevent cross contamination and stated the facility did not have a perineal care policy. The Administrator stated staff should sanitize hands and wear gloves before perineal care and remove gloves and sanitize hands when going from dirty to clean.
