Summary

The facility failed to electronically submit complete and accurate PBJ staffing information based on payroll and other verifiable and auditable data. Review of PBJ FY Quarter 2 2024 showed no RN hours reported on January 1, 2, 3, 9, 13, 14, March 20, and March 31, 2024, and no licensed nursing coverage for 24 hours a day on multiple dates in January, February, and March 2024. However, review of employee time cards showed RN coverage for 12 hours on the dates when no RN hours were reported, and showed licensed nursing coverage for 24 hours a day on the dates when 24-hour coverage was not reported. Review of PBJ FY Quarter 1 2025 showed no RN hours reported on October 2, November 15, 18, 20, and December 29, 2024, and no licensed nursing coverage for 24 hours a day on several dates in October, November, and December 2024. Employee time cards showed RN coverage for 12 hours on the dates when no RN hours were reported, and showed licensed nursing coverage for 24 hours a day on the dates when 24-hour coverage was not reported. Review of PBJ FY Quarter 2 2025 showed no licensed nursing coverage for 24 hours a day on January 1, 11, 18, February 16, and March 16 and 30, 2025, while employee time cards showed RN coverage for 12 hours on those dates. During interviews, the Administrator stated the facility had errors in the staffing data submitted, including not including agency staff, and acknowledged that the facility had reported inaccurate staffing information.
