Summary

The facility failed to provide necessary care and services to ensure that a resident's ability to perform activities of daily living (ADLs), specifically bathing/showering, did not diminish without a medical reason. A resident admitted with a history of pelvic fracture, chronic pain, and spinal stenosis required supervision or touch assistance from one person for bathing. Despite this, documentation showed that the resident received only two showers in November, with a gap of ten days without a shower from admission onward. The resident reported going almost two weeks without a shower and expressed a desire for more frequent bathing.
