Summary
The facility failed to ensure that a Registered Nurse (RN) was on duty for at least eight consecutive hours in a 24-hour period for one of the 21 days reviewed. Specifically, review of the facility's Nurse Staffing Reports showed that there was no RN coverage for any shift on one particular day. The facility's own policy, updated in March 2025, requires sufficient staffing with appropriate competencies to assure resident safety and well-being, taking into account census, acuity, and resident diagnoses.
