Summary

The facility failed to provide necessary assistance with activities of daily living (ADLs) to two residents who required help to maintain personal hygiene. For one resident with chronic medical conditions and a need for personal care assistance, observations revealed unshaven facial hair, greasy hair, and mild body odor. The resident reported missing a scheduled bath, stating he had not received a bath for four days and that this was not the first time his scheduled bath was missed.
