Summary
The facility failed to ensure that a Registered Nurse (RN) was on duty for at least 8 hours on one of the days reviewed for staffing requirements. Specifically, on 04/19/2025, the only RN present was the Director of Nursing (DON), who clocked in at 9:01 AM and clocked out at 11:13 AM, working less than the required 8 hours. Review of staffing records and time sheets confirmed that no other RN worked that day, and there was no documented evidence to show that an RN was present for the required duration.
