Summary

The facility failed to provide at least two baths per week for two of three residents reviewed, despite both residents being assessed as requiring substantial or maximal assistance for showering. Clinical record reviews showed that both residents were scheduled for showers twice weekly, but documentation revealed that the actual number of showers received was inconsistent and often less than scheduled. One resident, with intact cognition and a care plan indicating the need for one-person assistance, reported not receiving showers very often, and records confirmed missed showers over several months.
