Summary

The facility failed to ensure that a Registered Nurse (RN) was on duty for a minimum of eight consecutive hours each day, seven days a week, as required by regulation. A review of the 30-day staffing pattern revealed that there was no RN on duty for at least eight hours on several specific dates.
