Staff acknowledged that care plans should be updated with significant changes, such as new catheters, changes in transfer needs, or diet orders, but these updates were not made. The facility's policy requires timely and appropriate assessment and care plan updates, but this was not followed, resulting in care plans that did not accurately reflect residents' current needs.
