Summary
Surveyors observed that the facility failed to ensure a working ventilation system in 20 out of 28 occupied resident rooms on the 300 and 400 halls. During an environmental inspection, it was found that the ventilation systems in the bathrooms of these rooms did not function, as evidenced by the inability of the vents to draw a tissue square to the surface, indicating non-operation. The Maintenance Director confirmed the lack of ventilation draw in these bathrooms and was unable to provide documentation of the last operational check.
