Summary

A deficiency was identified when a resident, who had poor vision and limited hand function, reported not participating in activities since admission. The resident stated that staff did not offer or provide one-on-one visits or in-room activities, and expressed a desire for staff to visit her, as she spent most of her time in her room. Despite claims that the resident had participated in several activities, no documentation or records were provided to support this, and no activity assessment or evidence of one-on-one visits was available by the end of the survey.
