Summary

The facility failed to implement an adequate and effective pressure ulcer prevention and treatment program for two residents with significant risk factors and existing wounds. For one resident, who was cognitively impaired, dependent on staff for activities of daily living, and incontinent, there was no initial care plan for skin integrity upon admission, despite the presence of skin tears and an open area to the coccyx. Nursing documentation lacked detailed assessment, staging, or description of the wound, and pressure ulcer prevention interventions were not initiated in a timely manner. The resident's care plan included monitoring and documentation of wound status, but nursing notes and wound assessments frequently omitted measurements and detailed descriptions. Additionally, there was no PRN order for wound care in case the dressing needed to be changed outside of scheduled times, and hospice staff were responsible for dressing changes only on specific days. Facility policy required comprehensive assessment and documentation of all skin conditions, including location, stage, measurements, and presence of exudates or necrotic tissue, as well as regular physician evaluation and care plan updates. However, these requirements were not met for either resident, as evidenced by incomplete assessments, delayed interventions, and lack of proper wound tracking. Interviews with staff and review of records confirmed that wound measurements and documentation were not consistently performed, and care plans were not updated in a timely manner to address the residents' needs.
