The facility failed to maintain an effective infection prevention and control program, as evidenced by several deficiencies observed during the survey. The facility lacked a specific policy for cleaning the washing machines, relying instead on cleaning them when visibly soiled. These deficiencies collectively placed residents at risk for healthcare-associated cross-contamination and infections.
