Deficiency in Food Temperature and Palatability

East Haven Nursing & Rehabilitation CenterBronx, New York Survey Completed on 02-25-2025

Summary

The facility failed to provide food and drink that were palatable and at a safe and appetizing temperature, as observed during the Recertification and Complaint Survey. The deficiency was noted on Unit 4, where food served during lunch was found to have suboptimal temperatures and was not appetizing or palatable. The facility's policy requires hot foods to be maintained at 140 degrees Fahrenheit or above and cold foods at 41 degrees Fahrenheit or below. However, during dining observations, several residents reported that their meals were often served cold and unappetizing. Test trays conducted on Unit 4 revealed that the temperatures of various meal components were below the required standards, with hot foods ranging from 90 to 130 degrees Fahrenheit and cold foods not meeting the necessary cold temperature. Interviews with the Food Service Director and the Director for Dietary Services highlighted issues contributing to the deficiency. The Food Service Director acknowledged that food should be served at 135 degrees Fahrenheit or above for hot foods and cited food supply issues and improper preparation as reasons for the suboptimal meal quality. The Director for Dietary Services mentioned that the cold temperature of the elevators during winter could be a factor in the food arriving cold. Despite these issues, the facility's Administrator was unaware of any food-related complaints. The deficiency was cited under 10 NYCRR 415.14(d)(1)(2).

Short Summary

A facility failed to ensure meals were palatable and served at proper temperatures. Several residents reported that food was cold or could be warmer, and 15 confidential residents said meals were cold most of the time across all meal forms. Pureed Diet Preparation Not Followed
D
F0804 F804: Ensure food and drink is palatable, attractive, and at a safe and appetizing temperature.
Short Summary

Pureed diet food was not prepared according to the facility recipe for two residents. A dietary aide processed smothered pork chops and gravy in a food processor without referring to the available recipe, while stating he knew what to add to thin or thicken food as needed. The dietary manager said she was still educating staff on proper procedures, and the facility policy required puree foods to be smooth, soft, and homogeneous with specific preparation guidelines.

Short Summary

Cold Food Served on Tray Line and at Point of Service: Hot menu items were observed at low temperatures on the tray line, with pureed items tasting lukewarm and not palatable. A test tray sent to the unit was later measured at 115 degrees F for pork, 97 degrees F for green beans, and 100 degrees F for rice, and both the surveyor and the Culinary Development Specialist described the food as cold, bland, and not hot or warm. Residents reported the food is always cold, sometimes requires reheating that staff are too busy to provide, and that they buy outside food or snacks because the kitchen meals are not hot enough.
