Failure to Maintain Dryer Lint Compartments

Eskaton Village Care CenterCarmichael, California Survey Completed on 01-09-2025

Summary

The facility failed to safely operate the dryer for a census of 33 residents due to improper maintenance of the lint compartment. During an observation and interview with the laundry staff, it was found that the lint compartments of the dryers contained two thick layers of lint, which were not cleaned at the beginning of the shift. The laundry staff confirmed this oversight. Further review with the housekeeping supervisor revealed that the lint compartment log indicated an expectation for staff to clean the lint compartment every two hours. However, there was missing documentation of lint removal on seven occasions during the evenings in January 2025. The facility's policy required equipment to be ready for use at all times, and the failure to clean the lint compartment decreased the facility's potential to prevent a fire hazard.

E
F0908 F908: Keep all essential equipment working safely.
Short Summary

Kitchen equipment was not maintained in safe operating condition. A plate warmer was plugged in but only held room-temperature plates, and staff said it had not warmed plates since the prior week or earlier. A dishwasher temperature gauge was also faulty, reading 110F while a separate gauge showed the water was 122F during operation. The Maintenance Director said he had not been notified of either issue and found no June maintenance entries for the plate warmer or dishwasher gauge.

Kitchen Equipment Not Maintained in Working Order
F
F0908 F908: Keep all essential equipment working safely.
Short Summary

Kitchen equipment was not maintained in proper working order, affecting nearly all residents who received meals from the kitchen. Surveyors observed significant ice buildup in two reach-in freezers, including on food products, and found the garbage disposal covered. Dietary staff said the disposal had been broken for about a year, and the DM reported the freezers were constantly in defrost with possible door seal problems. Maintenance confirmed the disposal needed replacement and said approval was delayed because the facility was being sold.
