Interviews with the Director of Nursing confirmed that the facility did not thoroughly evaluate Resident 204's sacral pressure ulcer for changes and failed to implement interventions to prevent the development of Resident 1's pressure ulcer. The lack of consistent wound measurements and the improper use of protective devices contributed to the deficiencies identified in the care of these residents.
