Summary

The facility failed to provide services consistent with acceptable standards of practice for a resident when staff did not accurately assess and document the condition of a left heel wound. The resident, who had severe cognitive impairment and was dependent on staff for daily activities, was admitted with a wound to the left heel. Despite having orders for wound care and regular assessments, the facility staff did not complete a Braden assessment upon readmission and failed to document changes in the wound's condition over time.
