The Director of Nursing stated that a post-fall investigation should be completed within 20 minutes, involving multiple staff members, but acknowledged that assessments might be discarded if staff felt it was not a fall. This practice resulted in incomplete and inaccurate medical records, failing to meet the facility's policy and professional standards for maintaining resident-identifiable information.
