Failure to Update Resident's Care Plan with Current Code Status

Summary

The facility failed to ensure that the comprehensive care plan for a resident was reviewed and revised by the interdisciplinary team after each assessment. Specifically, the care plan did not reflect the resident's current code status. The resident, who was slightly cognitively impaired, had a documented code status of Do Not Resuscitate (DNR) in the physician orders and an Out of Hospital Do Not Resuscitate (OOH-DNR) form. The ADMIN and DON were unaware that the care plan for the resident indicated a Full Code status, despite the resident's current DNR status. The ADMIN stated that the change in code status should have been updated on the care plan immediately, and the ADON should have been trained to complete and update care plans accurately. The facility's policy on care plans requires that they be comprehensive, person-centered, and reflect the resident's current needs and conditions. The policy also mandates that care plans be reviewed and updated when there is a significant change in the resident's condition, such as a change in code status. The failure to update the care plan to reflect the resident's DNR status could place residents at risk of not receiving appropriate care to meet their current needs.
