The facility failed to ensure that 11 residents received their physician-ordered medications, leading to significant medication errors. The LPNUM admitted to being behind in the medication pass and failing to notify the Director of Nursing (DON) or the physician about the situation. The LPNUM was eventually relieved by the night Registered Nurse (RN), who discovered the unopened medications. The facility's failure to administer medications as prescribed and the lack of timely communication and reporting contributed to the significant medication errors observed during the survey.
