The facility staff failed to provide an ongoing program to support residents in their choice of activities based on comprehensive assessments and care plans. Resident #26, with diagnoses including hyperlipidemia and major depressive disorder, reported rarely participating in activities due to a lack of staff engagement and assistance with transportation to activities. An activity admission assessment was not completed for this resident. The facility's policy stated that the recreation department would provide an ongoing program based on comprehensive assessments and care plans, but this was not implemented for the residents mentioned. The Activities Director had not completed a certification program, and the facility administration did not address this issue when it was brought to their attention.
