The facility failed to ensure that Resident #111 received food prepared in a form designed to meet their individual needs, as required by their physician's order. Resident #111, who had diagnoses including Alzheimer's disease, gastro-esophageal reflux disease, and dysphagia, was ordered a regular diet with chopped consistency due to being edentulous and at risk for malnutrition.
