The TAR and Progress Notes for this resident from 07/01/24 to 07/07/24 also lacked documentation of tubing changes. Interviews with the Director of Nursing and nursing staff confirmed the weekly change policy and the lack of documentation for these residents. The deficiency was identified due to the absence of orders and documentation for the nebulizer tubing changes for both residents.
