The Nursing Home Reporting policy also stated that if events causing the allegation involve abuse, law enforcement must be called immediately, no later than two hours. The resident involved, R27, was admitted with paranoid schizophrenia with paranoia and delusions, dementia, depression, and anxiety. On 8/27/25, nursing notes documented that R27 asked a CNA, "Are you going to hit me like you did this morning?" and the allegation was reported to the nurse manager. The facility self-report stated that the CNA reported R27 accused her of hitting her during morning care, that the CNA was removed from the floor, and that the DON was notified of the allegation. The nurse manager and social worker later interviewed R27, who gave inconsistent responses, at times stating her CNAs were rough with her and at other times denying being hit or being unable to recall what happened. The record also showed a skin assessment finding a 2 x 2 cm dark purple bruise on R27's left calf, though it was described as not appearing new or suspicious. On 9/10/25, the DON stated the facility did not notify local law enforcement of R27's allegation of staff hitting her and acknowledged that a concern of a staff member hitting a resident is an allegation of abuse. The DON further stated the facility should have called local law enforcement per facility policy.
