Despite those documented choices, the resident’s code status preferences were not addressed in the care plan, were not included in the Point Click Care physician’s orders, and were not shown on the resident’s face sheet in Point Click Care. During interview, the DON confirmed the code status was not listed on the resident’s profile page and stated it should have been there; the DON also stated that in a code situation staff look first at the resident’s medical record profile page to determine code status. The DON then updated the resident’s code status on the Point Click Care profile page. During another interview, an RN selected a resident and identified the profile page as the place to find code status, matching the location that had been blank for Resident #6.
