The administrator and DON were informed of the findings.

On 10/14/2025, observations of Resident #4’s room at approximately 2:40 p.m. and 4:45 p.m. showed the incentive spirometer lying uncovered on top of the bedside table. A further observation on 10/15/2025 at approximately 8:20 a.m. again showed the incentive spirometer uncovered on the bedside table. During an interview on 10/14/2025, the resident stated that the incentive spirometer was used. On 10/15/2025, an LPN stated that the device should be covered to prevent contamination, and after observing the uncovered spirometer, stated that it should have been covered. The administrator and DON were informed of the findings on 10/15/2025.
