The facility failed to maintain and ensure the prevention and spread of infection when an LPN did not wear the required PPE during medication administration for a resident with a PEG tube. The facility policy on Enhanced Barrier Precautions stated that EBP include the use of gown and gloves during high-contact resident care activities for residents with wounds or indwelling medical devices. Resident #15 was admitted with diagnoses including Alzheimer's, dementia, and dysphagia, had severely impaired cognition on the MDS, and had a feeding tube. Physician orders included Enhanced Barrier Precautions and an order for alprazolam to be given via PEG tube for anxiety. During observation in the resident's room, the LPN prepared the alprazolam, entered the room, paused the enteral feeding, disconnected the tubing, checked placement, checked residual, and administered water and the medication through the PEG tube. The LPN wore gloves during the procedure but did not wear a gown. After the medication was given, the LPN removed the gloves, washed hands, and returned to the medication cart. When asked what should have been worn when giving medication through the PEG tube, the LPN stated gloves. The DON later stated that staff should wear gown and gloves when administering medication through a PEG tube.
