Surveyors found missing temp logs, temps above the required range, ice buildup, and undated food items in resident refrigerators, including a cognitively intact resident with Alzheimer’s disease, another with lung disease and osteoporosis, and a resident with severe cognitive impairment and dementia.
Unlabeled IV Tubing for Resident Receiving Antibiotics: A resident with cellulitis, lung disease, and HF was receiving IV cefepime and vancomycin, but the IV tubing was not dated. Surveyors observed empty antibiotic bags still hanging on the IV pole with tubing attached and no date labels, and the ADON confirmed the tubing sets had not been dated and she could not tell when they were last changed.
Opened Tuberculin Vial Not Dated in Medication Refrigerator: An LPN was observed with a medication refrigerator containing an opened multi-dose vial of TUBERSOL that was not dated with an open date. Manufacturer guidance and facility policy required the vial to be discarded 30 days after opening, and the LPN and DON both confirmed they did not know when it had been opened because it had not been dated.
A resident with dementia, ureteral calculous obstruction, and obstructive/reflux uropathy had an indwelling catheter, and staff failed to keep the catheter bag and tubing off the floor as required by facility policy. Surveyors repeatedly observed the drainage bag and tubing touching or lying on the floor, and an LPN and the DON confirmed the expectation that the bag and tubing be off the floor and attached to the bed frame to prevent UTIs.

During observation in the kitchen walk-in cooler with the Certified Dietary Manager (CDM), 1 gallon of whole milk and 1 gallon of buttermilk were open, undated, and available for resident use. The CDM confirmed the milk and buttermilk were open, unlabeled, and available for use. Facility policy required food items to be appropriately labeled and dated and foodservice equipment to be clean, sanitary, and in proper working order. The facility also failed to discard expired cold food items in the nourishment room. During observation with the CDM, 11 black cherry and mixed berry low fat yogurts and 5 whole milk 12-ounce bottles were found in the nourishment room refrigerator with expiration dates of 2/9/2026 and 2/7/2026, respectively, and were available for resident use. In the kitchen line prep cooking area, an oven had baked food particles and brown splatter inside, a convection oven had black and brown splatter on both inside doors and a sticky brown substance on the front legs, the floor beneath the convection oven had a greasy black substance, and a fryer contained food particles and grease splatters. The cleaning log showed the ovens were to be cleaned daily and the convection oven and deep fryer weekly, but documentation was missing for the oven and convection oven cleanings during the reviewed period, and the fryer was last cleaned on 2/9/2026.

Resident #114, who had diagnoses including Alzheimer’s Disease, Rheumatoid Arthritis, and Atrial Fibrillation and was cognitively intact per BIMS, had a refrigerator temperature log showing temperatures of 42 degrees on 4 days and no recorded temperatures on 8 days. During observation, the freezer compartment had a tray with a yellow substance and multiple small gray fuzzy-appearing splotches, and the refrigerator contained a cinnamon roll and a sandwich in undated ziplock bags. Resident #71, who had diagnoses including a lumbar vertebrae compression fracture, osteoporosis, and lung disease and was cognitively intact, also had no temperature entries on 8 days, and the refrigerator thermometer read 44 degrees with a cinnamon roll in an undated ziplock bag. Resident #11, who had dementia, epilepsy, and traumatic brain injury and severe cognitive impairment, had a large accumulation of thick ice build-up filling half of the freezer compartment and no temperature entries on 8 days. Resident #31, who had multiple sclerosis, hypothyroidism, and hypertension and was cognitively intact, also had no temperature entries on 8 days.
Unlabeled IV Tubing for Resident Receiving Antibiotics

The facility failed to provide for the safe, appropriate administration of IV fluids when it did not label the IV medication tubing for Resident #97. The resident was admitted with diagnoses including cellulitis of the lower limb, lung disease, and heart failure, and a BIMS assessment showed the resident scored 15, indicating cognitive intactness. Physician orders included cefepime 2 grams IV every 8 hours and vancomycin 1 gram IV every 12 hours for the resident. During an observation, the resident was sitting in a wheelchair at bedside with an IV pole nearby, and both antibiotic medication bags were empty after the last administration and still hanging from the pole. The tubing remained attached to the bags, and neither tubing set was labeled with the date they had been changed. The MAR showed the resident received vancomycin IV, and during an observation and interview, the ADON confirmed both IV tubing sets had not been dated and she could not see when they had last been changed or when they needed to be changed. A later observation showed the resident again sitting in a wheelchair at bedside with an IV medication hanging on the pole and infusing into the IV site in the right upper arm, and the IV medication tubing was still not dated.

During observation in the medication room with an LPN, the medication refrigerator contained 1 opened multi-dose vial of Tuberculin Purified Protein Derivative (TUBERSOL) with an expiration date of 7/2028, but the vial was not dated with an open date. The LPN stated she was unaware when the vial had been opened and confirmed it should be discarded 30 days after opening. The DON also confirmed the opened Tuberculin vial was not dated when opened and that the discard date was unknown because it had not been dated.
Indwelling Catheter Tubing and Bag Left on Floor

During interview, an LPN confirmed the urinary bag and tubing are to be off the floor to prevent potential UTIs, and the DON confirmed the expectation is for the catheter bag and tubing to be off the floor and attached to the bed frame to prevent UTIs.
